The avatar folder you mentioned? Is that yourboard/images/avatars/ ?I used WinMerge ignoring dates, using full contents. The clean 3.3.11 site has the install folder and everything under that. The live system has Cache, ext, cgi_bin, and manager_mine.php under the avatar folder.

That should contain /gallery, /upload and index.htm.
The .csv you uploaded doesn't help.
You need to compare a fresh 3.3.11 fileset with the 3.3.11 fileset you're using on you host.
Disregard timestamp and only list the differences. (also no point in checking the /cache as the contents of that are created on the fly: so you might as well empty that, but for .htaccess and index.htm)
Also don't bother to check the /files (as long as it contains .htaccess and index.htm)
